Epimacular Brachytherapy–a Promising Treatment Option for Patients With Neovascular AMD

چکیده
JULY/AUGUST 2010 ADVANCED OCULAR CARE 33 R adiation therapy has been evaluated extensively as a treatment for choroidal neovascularization (CNV) due to age-related macular degeneration (AMD) because of its antiangiogenic, antifibrotic, and anti-inflammatory properties.1-3 Historically, radiation for AMD has been delivered with two broad approaches: (1) radiotherapy with external beam (megavoltage X-rays, proton beam) and (2) brachytherapy with radioactive isotopes (strontium-90, palladium-103).4-11 Radiation therapy was never widely adopted by the retinal community because it did not demonstrate a reproducible positive effect on the visual acuity in patients with AMD.7,8 Radiation therapy also had inconclusive, beneficial anatomic findings8,11 and was associated with complications in a small number of patients.6 To date, the results of radiation therapy in AMD have been disappointing, and its use has been supplanted by vascular endothelial growth factor (VEGF) inhibitors.12-14

Radiation therapy for neovascular age-related macular degeneration
Antivascular endothelial growth factor (anti-VEGF) therapies represent the standard of care for most patients presenting with neovascular (wet) age-related macular degeneration (neovascular AMD). Anti-VEGF drugs require repeated injections and impose a considerable burden of care, and not all patients respond. Radiation targets the proliferating cells that cause neovascular AMD, including fibro...

Age-related macular degeneration (AMD) is considered the most common cause of blindness in the over-60 age group in developed countries. There are basically two forms of presentation: geographic (dry or atrophic) and wet (neovascular or exudative). Geographic atrophy accounts for approximately 85%-90% of ophthalmic frames and leads to a progressive degeneration of the retinal pigment epithelium...

In developed countries, age-related macular degeneration (AMD) is the leading cause of irreversible blindness in individuals over the age of 65 years. Vascular endothelial growth factor (VEGF) plays a vital role in the formation of neovascular AMD. VEGF regulates angiogenesis, enhances vascular permeability, and drives the formation of choroidal neovascularization. As a result of the introducti...

INTRODUCTION Neovascular age-related macular degeneration (AMD) is the leading cause of severe, irreversible visual impairment in people over 60 years of age. Neovascular AMD is characterized by abnormal growth of blood vessels under the retina, specifically the macula. These vessels leak blood and fluids, damaging the retina and its photoreceptors, resulting in permanent loss of central vision...
متن کاملEarly detection and treatment of neovascular age-related macular degeneration.
BACKGROUND The neovascular form of age-related macular degeneration (AMD) can rapidly lead to severe loss of central vision and adversely affect the patient's quality of life. During the 1990s the only proven treatment for neovascular AMD was laser photocoagulation. Only a minority of patients are eligible to receive this treatment, however, and the treatment itself can cause acute retinal dama...
